Mission Supports Program Information

To cover direct necessity expenses for a phone, transportation, a safe place to sleep with a shower for the night before an important appointment, and court and county fee expenses, among many other things where your contribution will go directly to those we serve.

Through a collaborative and creative solution-based approach, we utilize sustainable and diversified resources to be the leading model in serving people with Intellectual and Development Disabilities (I/DD) experiencing housing insecurity to navigate supports on the path to stability.

Transitional Case Management for adults who need side by side assistance to obtain necessary benefits specific to an intellectual and developmental disability or delay

Denver Mill Levy services

Since 2017 CFPD has contracted with Rocky Mountain Human Services' Denver Mill Levy program to provide transitional case management services to people with intellectual/developmental disabilities (I/DD) who are homeless or at-risk of homelessness in Denver County. Eligible participants receive close assistance to prepare for, apply and obtain an official determination that they are eligible for Colorado HCBS IDD or SLS programs through Rocky Mountain Human Services.

Through the process of pursuing determination for services, CFPD's Mission Supports program will assist with obtaining an up to date state ID, Social Security card and active Medicaid medical coverage, an active or pending Social Security disability claim, and will also attend court hearings and other meetings that are necessary to keep the person on a path to obtaining disability-specific state and county services and permanent housing.

 

Wrap Around services

Thanks to 2020 funding from the Denver Foundation, Mission Supports is able to continue transitional case management for those individuals who are not determined eligible by RMHS, have obtained permanent housing prior to determination decision or have settled in a shelter outside of Denver, or do not meet other requirements to utilize Denver's Mill Levy funding.  Through our Wrap Around service we also can take referrals from shelters and providers outside of Denver county and can provide assistance with pursuing permanent disability specific services and housing for those with an intellectual and developmental disability or delay.

 

Is this the service for you?

  • Diagnosed with an Intellectual or Developmental Disability (I/DD) prior to age 22. 
  • Special Education was primary school experience or was not able to finish high school. 
  • Diagnosed or tested for Autism Spectrum Disorder, Down Syndrome, Fragile X, Fetal Alcohol Syndrome, child onset Epilepsy, Cerebral Palsy, Developmental Delay, Traumatic Brain Injury prior to 22, Mental Retardation or Pervasive Developmental Disorder.
